"End of Program" Feedback Survey

The Nutrition Institute of the University of Bridgeport End of Program Feedback Survey is designed to collect information about students’ perceptions of the quality of the Nutrition Program at the time that they are completing the program. The data collected from the survey will be used for program evaluation and improvement and to fulfill accreditation requirements. Please help us by responding to the questions as candidly as possible. The survey does not ask for any identifying information.  All responses will remain confidential. Your responses to this survey will be aggregated with other students’ responses, and reviewed by the Nutrition Institute in aggregated form.  The survey should take approximately ten to fifteen minutes to complete.
